Incident Report Forms

The following report form needs to be filled out by the faculty member when a cheating incident has been witnessed. The completed form is then submitted to the appropriate department head(s) and to appropriate dean(s). A copy of the report is also to be submitted to Shara Gibbons shara.gibbons@usu.edu, so she can document the incident.
